It seems to randomly replace any other Meg when it spawns. Very low chance, possibly 1 in 10,000. The one I found was in the Devils Roar far away from fog. I was on an Athena quest at the time. I knew the shrouded ghost was a Meg variant but had no idea how rare/legendary it was (I wasn't really involved with the community much at the time).
